What to bring to your appointment

New and existing patients will need to bring the following items to each appointment:

Drivers license and/or picture ID
Insurance card (if insured)
Co-Payment (cash or credit card)
Medication list and immunization card

If this is your first time visiting Unity Health on Main you will need to download the following forms to fill out and bring them with you to your first appointment:

Authorization to Release Medical Records
